This subject examines the philosophy of primary health care and the application of the principles in contemporary health systems. Using diabetes as an example, the subject explores systems and models of care that can be applied to prevent and manage a chronic disease. These include the Chronic Care Model, the Enhanced Primary Care Program, self management and the National Integrated Diabetes Program and Clinical Information Strategies. Throughout this subject, primary health care concepts are used as a theoretical backdrop for understanding these programs. The barriers and enablers for optimal health care delivery are explored.This subject is accredited by the Australian Diabetes Educators Association (ADEA) and aligns with its standards and guidelines.
